DVA Community Nursing Programs Sydney

Oxley Home Care are currently contracted by the Department of Veteran Affairs (DVA) to provide DVA Community Nursing Services to clients living in Sydney Metropolitan area.
DVA provides veterans and war widows/widowers (entitled persons) with access to a range of quality health care and related services, including community nursing services, at DVA’s expense.

The DVA Community Nursing Program provides home community nursing services for entitled persons to meet their assessed clinical and personal care needs.

As a DVA contracted community nursing provider, Oxley Home Care’s aim is to enhance the independence and health outcomes of the entitled person by avoiding early admission to hospital and/or residential care through access to community nursing services. These community nursing services are delivered by registered nurses, enrolled nurses and nursing support staff.

Eligibility for DVA Community Nursing Program

If you are a Gold Card Holder or Veteran White Card holder living at home and have been assessed as needing DVA Community Nursing home care assistance, you may be eligible for support through DVA Community Nursing.

To receive DVA Community Nursing, the veteran, war widow/widower must receive a referral from their Doctor, the Hospital Social Worker or Hospital Discharge Planner.

Following a referral from a Doctor, Hospital Social Worker or Hospital Discharge Planner, our Registered Nurse will arrange a time to complete a comprehensive assessment.

What level of assistance is available?

The duration and frequency of support will be dependent on the outcome of the comprehensive assessment. All Veterans are also eligible to Veteran Home Care services and Government Home Care Packages and Private Care, which can be used to top up services funded through DVA.

To be eligible for access to a Home Care Package you require a comprehensive assessment by your local Age Care Assessment Team (ACAT). This needs to be arranged by contacting My Aged Care on 1800 200 422 or visit My Aged Care website www.myagedcare.gov.au

 

What type of support will I get?

The types of DVA Community Nursing services that may be provided include:

  • Case Management
  • Personal Care
  • Wound Care
  • Incontinence management
  • Medication
  • Chronic disease management
  • Palliative care
  • Pain management
  • Making referrals to other Allied Health, and other community care services as needed

Funding

DVA Community Nursing is funded by the Australian Government through the Department of Veterans Affairs. There is no cost to program recipients.

What happens if my needs are greater than the support I am getting?

All Veterans are also eligible for Government Home Care Packages. For more information about the funding available through this package, visit their My Aged Care website.
