Jean is 84 years old and suffers from Parkinson’s disease. She has been in hospital for 6 weeks and is now ready to go home. Jean lives alone in a Self Care Unit in a Retirement Village; her adult children are supportive and phone regularly but do not live close by. Jean’s daughter is very concerned her Mother is still not very strong and feels she will need some assistance with personal care, shopping and general housekeeping for a few weeks or until she regains her strength.
Jean’s daughter contacts Oxley Home Care and our Care Coordinator visited Jean at the hospital. The Coordinator talked to the discharge planning team and Jean, and they discussed a plan of services to meet Jeans needs. Our Coordinator organized a Care Worker to pick Jean up from hospital and transport her home; the Care Worker unpacked Jean’s bags and made a cup of tea before going shopping for food. The Care Workers came in daily for the first 2 weeks to help Jean with her shower and get dressed. They changed the bed linen and helped with light domestic tasks and shopping. Over this time Jean felt stronger and more capable of doing a little more herself. The personal care services were then dropped back to 3 times per week and Jean now enjoys going to the shops with the Care Worker and stopping for a coffee on the way home. After 6 weeks Jean felt back to normal and didn’t require the personal care support services anymore but has enjoyed the social outings and shopping with the care worker and has decided to keep these in place.
